Create Sub Form based on various Categories

Good afternoon everyone.  I was wondering if anyone had information/documentation on how to create/call a sub form based on Category types?  For example.  If a Category is Software->Software Request

call a subform that would ask them for the title of the software, special install instructions, etc.  I would only want certain forms called by certain categories.
